They feel good when you can adjust the appropriate tension. When they are slightly looser, they are left open enough at the outer edge that they come off easily.

Quite OK. Despite the adjustability, the clamps stay pretty badly on the nipples and even worse on the clitoris, but e.g. when lying on your back they stay in place quite well.
